Notice for Pet Care Insurance Online Enrolment
You can enrol online for your pet if it can meet all the below criteria:
1. Cats(all species) or dogs(Chihuahua, Corgi, Golden Retriever, Pekingese, Pomeranian, Poodle, Pug, Schnauzer, Shiba Inu, Shih Tzu, Siberian Husky or Yorkshire Terrier).
2. Aged between 1 and 4
3. No treatment required due to accident or illness in the last 90 days, excluding general check-up or preventive vaccinations
4. Never received any surgical operation before, except desexualisation.
5. No record of attacking/biting any person/animal or manifesting any aggressive/vicious tendency in the last 5 years.
6. Never engaged in any business or commercial activities.
7. Never suffered from any physical defects or infirmities.
In case the above criteria cannot be met, please call 3608 2930 for filing a written application.

Online application is only applicable to pet aged from 1 year to 4 years and 11 months. If the age of your pet is out of this range, please send in a completed application form for underwriting process.
1. Has your pet received or required any treatment for an accident or illness in the last 90 days, except general checkup and preventive vaccinations?

3. Has your pet ever attacked or bitten any person or other animal, or does it have any aggressive or vicious tendency in the past 5 years?
4. Is your pet being used for or in connection with any trade or business?
5. Does your pet suffer from any physical defects or infirmities?
